These phrases express mediation, instrumentality, or the agent through which something is accomplished. They answer the question: Per tramite di chi? Attraverso quale mezzo? (Through whom? By what means?).

These phrases express the cause, reason, or motivation behind an action or event. They answer the question: Per quale motivo? A causa di chi/che cosa? (For what reason? Because of whom/what?).
